From what I know it's only the nipple, it'll get puffy (you'll really know when/if this happens) and most people say it gets sensitive/itchy to touch. Once gyno starts developing you can usually feel the lumps under the nipple notably. With me personally I always get puffy nipples but they've never been sensitive or itchy, and I've never gotten any lumps, so I assume it's never been gyno, though it always seems puffy while on cycle it goes back to normal when it's cold or I cycle off. I do use adex, but only either a 1/8th tab twice a week or 1/4th of a tab once a week. My nipples stay puffy but if I take enough to make the puffiness go away it always kills my sex drive, my mood and my joints. If you are getting gyno most people say you can shrink it back down in the early stages with letro at a mild dose
